Colorado Health Foundation’s Advance Care Planning Funding Opportunity to Reach More Rural Communities

In partnership with seven new organizations, the Colorado Health Foundation is expanding the Advance Care Planning funding opportunity’s geographic reach to encompass more rural regions of the state.

Grantees awarded for the third and final cycle of funding include: Community Health Action Coalition, University of Colorado Foundation, Community Health Partnership, Sangre de Cristo Hospice and Palliative Care, Catholic Health Initiatives Colorado Foundation, Rocky Mountain Rural Health and the Colorado Gerontological Society.

The third cohort is a mix of urban and rural nonprofit organizations – both large and small in size. In addition to supporting Front Range organizations, grants were awarded in the Southwest, Southern, Southeast and Eastern Plains regions of the state. The majority of these projects are community engagement initiatives utilizing partnership-oriented approaches to engage and educate residents and health care providers in advance care planning.

The three-year initiative is designed to accelerate existing advance care planning efforts in Colorado where feasible and to support innovative demonstration projects that have the goal of increasing individual and family health engagement through advance care planning. In total, 17 organizations have received funding to support their work under this initiative. The three grantee cohorts will focus their activities in one or more advance care planning domains, including engagement, education, system infrastructure and continuous quality improvement.

“We are pleased to extend the reach of this initiative to a geographically diverse array of communities across the state,” said Dara Hessee, senior program officer at the Colorado Health Foundation. “As a whole, these efforts will strengthen upstream advance care planning conversations and help ensure that Coloradans are more engaged in their health and receiving care that aligns with their values, goals and informed preferences.”
